Access Clinical Support Services

HCP’s dedicated Garden City staff support the clinical and administrative needs of our entire network.

Our clinical teams assist you, your staff and your patients to ensure access to quality cost-effective care at all levels.

Two “Campus” teams cover our geography to ensure we can support our provider’s needs.

  • The West Campus supports all of New York City, except for Queens, and Westchester County.
  • The East Campus supports Queens and all of Long Island.

Each campus consists of experienced administrators and physicians, along with teams of clinicians including registered nurses, social workers, care coordinators, physician reviewers and pharmacists, all of whom work collaboratively to support practices’ needs.  These teams proactively identify members who would benefit from their support, with a focus on more complex and chronic needs.

Working collaboratively as interdisciplinary care teams, each campus is able to assess individual patient needs and develop care plans to comprehensively address each patient’s clinical and psychosocial needs.

Providing administrative support is the role of HCP‘s provider relations representatives, who also serve as liaisons to the clinical staff when necessary. Each campus has its own provider relations staff who work in an integrated way with the clinical team.

Our campus teams drive much of the patient specific activity we identify from our comprehensive data mart. We look to you, our provider network, to help identify patients early and look forward to receiving referrals from you and your staff.
