What role does HCP play in my health care?
HealthCare Partners (HCP) is an Independent Practice Association (IPA) made up of thousands of Primary Care and Specialist doctors throughout the Greater NY Metropolitan Area.
We are led by physicians who help doctors remain independent so that they can deliver great care to their patients. The IPA provides clinical and administrative support to help physicians focus their attention on you, their patient.
We also work with your insurance company to help ensure you have access to all the health benefits you need. Because of our long history of success, your insurance company has partnered with us to help coordinate your care and to also perform some administrative services for them.
How does HCP help coordinate your care?
We’re here to help you navigate your health and connect you with the care, services, and resources you need. Here are just a few ways we can support you:
- Schedule appointments with your physicians
- Recommend specialists in our network who require no referral
- Send reminders about preventive measures you need to keep you healthy
- Provide care coordination when you transition home from a hospital visit – this may include helping you or your caregiver understand what you need to do, explain medications, educate you on your condition, create a case management care plan, or set up follow-up appointments
- Help you access resources in your community that offer food and nutrition, housing, transportation, etc.
- Connect you to a behavioral health care manager
- Our pharmacists can talk with you and/or your provider about your medications and doses
- Locate an urgent care center near you
- Help to obtain health information about medical conditions that may be affecting you or your loved ones

If you need assistance with any of the following, please contact your primary insurance company listed on your ID card:
- Updates to personal or demographic information (i.e., address, name change, phone number, date of birth, etc.)
- Adding or removing dependents from your plan (newborn, aged out dependent, change of marital status, etc.)
- Selecting or changing your Primary Care Physician (PCP)
- Grievances/disputes regarding claims processing (payment or denials), denials for prior approval or referrals
- Copies or replacements of your member identification card
- Questions regarding your benefits and/or copays/coinsurance
- Copies of benefit summaries and/member handbooks
- Update your telephone number
- General complaints and/or grievances about copayments, service, or other patient experience
Important Information on Coverage Decisions
Each day, HealthCare Partners Utilization Management (UM) Department makes decisions on many health insurance claims. These decisions are based only on the appropriateness of care and the existence of coverage.
HealthCare Partners does not reward providers or other individuals for issuing denials of coverage and does not offer financial incentives to UM staff based on decisions that promote underutilization of services.
