Special Needs Plan (SNP) Model of Care (MoC) Training Period: Jan 1, 2026-Dec 31, 2028

NOTICE | APRIL 3, 2026

All Medicare providers are required to complete the Special Needs Plan (SNP) Model of Care (MOC) training* for each of the dual-eligible SNPs in which they participate, as mandated by the Centers for Medicare & Medicaid Services (CMS). Providers in the VIP Bold and Reserve Networks must complete the 2026-2028 EmblemHealth SNP MOC provider training.

To satisfy this requirement, providers must complete the training and submit the applicable attestation by April 15, 2026. You will receive periodic reminders during the 2026-2028 training period until you fulfill this requirement.

*Providers who completed EmblemHealth’s SNP MOC training conducted in 2024 are also deemed compliant for 2025 SNP MOC provider training requirements. Please note this does not extend to required provider SNP MOC trainings for years 2026 through 2028.

Completing the Attestation

To ensure compliance with this federal requirement:

  1. Go to emblemhealth.com/snp-moc
  2. Review the 2026-2028 EmblemHealth SNP MOC provider training.
  3. Check the box for either Individual or Group Attestation.
  4. Check the box at the attestation statement and fill in the required fields.
    Note: For Group Attestation – a separate attestation is required for each group/organization tax identification number (TIN).
  5. Click “SUBMIT,” and you will receive an email confirmation.
