Experienced administrators and physicians, along with teams of clinicians including registered nurses, social workers, care coordinators, physician reviewers, and pharmacists, work collaboratively to support your practice’s needs. These teams proactively identify members who would benefit from their support, with a focus on more complex and chronic needs.
Working collaboratively as interdisciplinary care teams, they assess individual patient needs and develop care plans to comprehensively address each patient’s clinical and psychosocial needs.
Providing administrative support is the role of HCP‘s provider relations representatives, who also serve as liaisons to the clinical staff when necessary.
