We are pleased to introduce Care Assist Referrals, designed to make it easier than ever for your patients to find a specialist or covered services like home care, PT, and durable medical equipment.
Care Assist Referrals
When your patient requires specialty care or services, your office staff can submit the request through EZ-Net. Simply enter the patient’s information, diagnosis, reason for the referral, desired specialty or service type, and include any relevant clinical notes or documentation. If you would prefer, you can contact our Customer Service team at (516) 394-5639 for assistance in creating the referral. Once submitted, an HCP team member will coordinate directly with your patient to ensure that they are scheduled with a trusted in-network specialist or service provider.
This service is currently available for HCP-managed EmblemHealth members. More plans will be announced in the near future.
