At HCP, a team of community based Member Advocates works in the field in support of our case management staff to assist in identifying barriers to care to better patient outcomes.
Acting as the eyes and ears for our internal clinical management teams, our trained and skilled Advocates meet with patients face to face in their home environment.
Member Advocates offer culturally sensitive home based outreach, evaluate patient social needs such as food adequacy, financial capacity and transportation means, deliver case management and provider directed health education materials, help bridge connections to community resources, and assist with making doctor’s appointments, among the many valuable services they can support.
Member Advocates ensure that our most vulnerable populations gain access to the many available Community Based Organizations and the resources they offer.

Member Advocate (MA) Referral Process
Suggested Referral Criteria
Medically complex patients with one or more poorly controlled chronic medical conditions
Frequent preventable Emergency Department visits
Takes more than 10 medications
Unstable housing or other social determinants of health
Difficulty accessing health care services
Lack of support in home or community
Poor understanding of disease and/or treatment (low health literacy, labeled “non-compliant”)
Provider is unable to contact the patient
Patient is not engaged in own care
Language or other social barriers
Newly diagnosed chronic conditions
